Affinity is currently In-Network with:

• CIGNA

• Anthem Blue Cross Blue Shield (PPO plans)

• CareFirst, including Federal plans (PPO plans)

Please note:

• We are typically NOT in network with the HMO plans or Anthem Healthkeepers.

• *We are NOT in network with Medicare or Medicaid plans.

• Please contact your insurance company to see if Christiane Davenport, NP or Bethany Morrell, NP are In-Network.

• If our in-network plans above are not your PRIMARY insurance, you will need to pay upfront and self submit for reimbursement.

FEES FOR TREATMENT:

Affinity also accepts Out-of-Network (self-pay) clients. You will be provided with an invoice (called a “superbill”) that you may submit to your insurance company for out of network benefits/reimbursement.

You may check with your insurance to see what they will reimburse you for “out of network” providers for the following CPT codes:

• Initial Full Psychiatric evaluation - (CPT codes 99204/90838 or 99205): $350

• Follow-up Visits (CPT codes 99214 & 90833/90836): $195 - $230

CANCELLATION /NO SHOW FEES: 

A minimum of 24 hours notice is required to cancel a scheduled appointment. Otherwise, a No-Show fee of $100 will be charged.
