FEES FOR TREATMENT:

Affinity also accepts Out-of-Network (self-pay) clients. You will be provided with an invoice that you may submit to your insurance company for reimbursement.

You may check with your insurance to see what they will reimburse you for “out of network” providers for the following CPT codes:

• Initial Full Psychiatric evaluation - (CPT codes 99204/90836 or 99205): $350

• Follow-up Visits (CPT codes 99214 & 90833/90836): $195 - $230

• For more detailed information on “Out of Network Costs” : GOOD FAITH ESTIMATE

CANCELLATION /NO SHOW FEES: 

A minimum of 24 hours notice is required to cancel a scheduled appointment. Otherwise, a No-Show fee of $50 will be charged.
