What insurance companies is Affinity in network with? 

Cigna, Anthem BCBS (PPO plans), and CareFirst (PPO and Federal plans)

 

How do I confirm if Affinity is an IN-network provider? 

Please call your insurance company/ check their website to determine if your provider is listed as an in-network provider with your specific insurance plan. 

 

Can I be a patient with Affinity if I do not have insurance or if Affinity does not take my insurance?

 Absolutely! We have many Self-Pay patients we work with and are happy to see you.

 

How much are the Self-Pay Fees? 

Initial visits are $350 and follow up visits $195.

 

How much will be due on the day of my appointment? 

  • If Affinity is in-network, you are responsible for your co-pay on the day of your visit.

  • For Self-Pay patients, payment is due in full on the day of your appointment.

 

What do I need to complete for my first appointment? 

  • You will need to complete the Intake paperwork 48 hours prior to your appointment.

  • This ensures we have time to verify your insurance and complete your chart.

  • Kindly remember to upload your driver’s license and insurance card.

 

What happens if I do not submit my paperwork on time? 

If paperwork is not completed within 48 hours prior to your appointment, unfortunately, your initial appointment may need to be rescheduled.

 

How long does the initial paperwork take to complete?  

The paperwork will take you approx. 30 minutes to complete.

  

How long will the Initial visit last? 

Your initial visit will typically last 60-75 minutes.

 

How long will follow up visits last? 

Your follow up visits will typically last 20-30 minutes.  

 

Do I need to have In-person visits or can I see my provider via telemedicine? 

  • At this time, Virginia law allows ALL patients to be seen either in-person OR via telemedicine.

  • In the future, however, Virginia law may revert back to pre-pandemic laws, and require an initial in-person visit for any patient being prescribed a controlled medication (stimulants, benzodiazepines, etc).

 

Do you perform ADHD testing? 

  • If you meet criteria for ADHD, we offer in-office computerized testing to support an ADHD diagnosis. 

  • However, some patients may require full neuropsychological testing with a psychologist, and would be referred.

     

If I have ADHD, but do not have prior records, will I be prescribed a stimulant? 

No, Patients will not be prescribed a controlled stimulant for ADHD unless they have ONE of the following:

1) a letter documenting your ADHD diagnosis from their prior Psychiatrist or Psych NP

2) neuropsychological testing documentation

3) an evaluation with our provider and completion of our in-office computerized testing (TOVA)

 

How often are controlled (ADHD or other) medications prescribed? 

You must have an appointment every 30 days to receive a prescription for any controlled substance, including stimulant medications.

 

How often will I need to see my provider? 

  • New patients are seen back after 2 weeks, and then monthly until their symptoms are stable

  • Patients on controlled substances will be seen every 30 days.

  • All others will be seen every 3 to 4 months.

Will my Provider order labs/bloodwork?

Yes.

Comprehensive labs are encouraged and will be ordered for you at your initial visit unless you have had these completed already with another provider in the past year.
